Many of them used as a kind of brand, names like "PZL-city/district" - where factory was placed. So if we list the Mi-2 helicopters as "PZL-Świdnik" and some others as "PZL-Mielec", I guess it should be PZL-Okęcie in this case too. This source - http://www.abpic.co.uk/photo/1008644/ calls SP-FWD "104 Wilga 35A" - without M.
